The application of scientific methodology, knowledge and experience to investigate an engineering failure, incident or accident with the aim of identification of mechanism(s) and root cause(s) of the failure, incident or accident; and provide recommendation of countermeasure(s) to prevent similar failure, incident or accident or minimize the failure rate. In addition, the investigated failure, incident or accident has been related to or may be related to a litigation matter; and the investigator may be engaged as expert witness to testify his investigation in a court trial.

Incident: an unexpected event that may result in loss of production and property damage, but does not result in an injury or illness of people and/or other living things;

Accident: an unexpected event that may result in loss of production and property damage, and does result in an injury or illness of people and/or other living things.
